We Accept Most Major Insurances

Prior to calling our office, it may be helpful to review your specific coverage plan offered by your provider. This should indicate which services are covered under your plan, when a copay may be required, and any additional information you may need to know. For questions about copay, coinsurance, deductible, or non-covered services, contact your coverage provider.

We accept most major insurances. We do not see Medicare, Medicare replacement policies, Wellpoint, Molina, or Iowa Total Care patients. We apologize for any inconvenience this may cause. Please contact our office staff to determine if we participate with your insurance and for any other insurance or billing questions.

We offer cash discounts for uninsured individuals. Please contact us ahead of time so we can provide you with all the information you need.
